The EuroQol Group has a proxy script for children and adolescents aged 7-12 years:

Proxy version 2: Asking the proxy to rate how he/she (the proxy) thinks the child would rate his/her own HRQoL if he/she (the patient) was able to communicate it (proxy 2). Proxy 2 is available in the following language:
