These are a standard set of questions concerning socio-demographic variables for use with the EQ-5D valuation questionnaire. A modified version can be used with the EQ-5D self-report questionnaire (often referred to as page 4).

Users should note that this page is optional. If users do decide to use the questions, they should also note that the questions on employment and education (Q6, Q7 and Q8) may not be comparable across countries. The questions were translated specifically to harmonize with the employment categories and education system in the country concerned.
